Congratulations to North Lewisham Primary Care Network (PCN) and Red Ribbon Living Well for winning the HSJ Primary and Community Care Innovation of the Year Award!
Their award-winning project, Health Equity Partnership: A symbiotic approach to tackling health inequalities, has made a real difference in our community by improving access to health services and reducing health inequalities.
Key achievements include:
- Health Equity & Wellbeing Champions – Local champions trained to identify health needs in vulnerable communities.
- Community Health Hubs – Nine one-stop hubs delivered NHS health checks and specialist screenings to 678 people, working with voluntary sector partners to bring care closer to home.
- HIV Stigma Training – Co-designed education sessions and animated resources to reduce HIV-related stigma in primary care.
This award highlights the power of collaboration to improve health and wellbeing for North Lewisham residents. Well done to everyone involved!
